Below are questionnaires specific to legal cases I handle. The questionnaires are designed to be completed on line and sent to my office prior to your first appointment. This helps us evaluate your case faster and thus helps keep our prices low. We will use this data to create a temporary file to store future communications and notes. It helps to provide us with as much information as you can about why you are seeking a lawyer, what your situation is and how quickly you need action. And if you have a dispute with an individual or company, please provide us with the name of this person or entity. Upon receipt we will contact you to set up a time for a phone consultation which, depending on the legal action needed, may require payment of a consultation fee.
The Uncontested Divorce questionnaire is for clients who have been separated a year, divided their marital properties, and ready to finalize their divorce. From this we generate divorce forms which can be processed anywhere in the State of North Carolina. The Limited Driving Privilege (LDP) questionnaire lets us to produce an LDP form for your DWI case to be used, again, anywhere in North Carolina. The General Case, Bankruptcy, DWI and Will questionnaires let us start a file to store any items you send us to review. If you are completing the DWI questionnaire I also recommend you complete the LDP questionnaire as well.
But again, simply submitting a questionnaire does not make you our client. We want your business but we still must reserve the right to decline employment if the circumstances warrant such action.
