Below are questionnaires specific to legal cases I handle. The questionnaires are designed to be completed on line and sent to my office prior to your first appointment. The Uncontested Divorce questionnaire is for clients who have been separated a year, divided their marital properties, and ready to finalize their divorce. From this we generate divorce forms which can be processed anywhere in the State of North Carolina. The Limited Driving Privilege questionnaire lets us to produce an LDP form for your DWI case quickly. In all cases completing the questionnaire helps us evaluate your case faster and thus helps keep our prices low. But again, simply submitting a questionnaire does not make you our client. We still reserve the right to decline employment if the circumstances warrant such action.
